Presenter: Body

Dr. Jeffrey Sommer moved to NJ after graduating with a Doctor of Physical Therapy degree in 2004 from Daemen College in Amherst, New York. As a Board Certified Orthopedic Physical Therapy Specialist, certified by the American Board of Physical Therapy Specialties, Dr. Sommer has in-depth knowledge of orthopedic injuries and surgeries, and a philosophy driven by an evidence-based model designed to facilitate the fastest return to function. He joined VNA Health Group in 2014, after selling his private practice. His work is currently focused on improving the delivery model of orthopedic, cardiac, and would care for the home care population.

Presenter: Mind
Dr. Shira Goldberg currently serves as the VNA Health Group's Medical Director for Hospice and Palliative Care for Monmouth Medical Center in Long Branch, New Jersey. She is a graduate of the Icahn School of Medicine class of 2004. She finished her residency in Internal Medicine at Robert Wood Johnson University Hospital in 2007. She is board certified in Internal Medicine, Geriatric Medicine, and Hospice and Palliative Medicine. After working in the field as a Primary Care Geriatrician since 2010, she recently joined the VNA Health Group to expand their clinical team in Palliative Care and Hospice. In her capacity as a clinician-educator at Monmouth Medical Center, she is the sole consultant for Palliative care and serves as the Medical Director for the inpatient hospice program. Apart from her clinical duties, Shira's passion is education. She enjoys exploring new and innovative ways to teach the next eneration of the clinical care team. She tries to focus on the importance of understanding and incorporating the principles of Palliative care into their clinical thought process.

Presenter: Spirit
Louis Monticchio, MTh, BCCC, is the Director of Pastoral Care and VNA Health Group Hospice and a board certified clinical chaplain. He is responsible for the pastoral care discipline that is providing spiritual and religious support for patients, families and staff. Lou leads a team of hospice chaplains and spiritual care counselors that collaborate actively with medical and social service professionals in providing a holistic healing presence at end of life. He is a commissioned Lay Ecclesial Minister in the Diocese of Trenton and active in pastoral leadership ministry in his local parish community in Hamilton, New Jersey.
